By filling out this form, the Scott County Auditor's Office will mail out an absentee ballot request form to the address provided. The completd request form will need to be returned prior to the 5:00 p.m. deadline to the Auditor's Office in person or by mail to the Scott County Auditor's Office, 600 W. 4th St., Davenport, IA 52801-1003. You can alternatively print an absentee ballot request form from your home and return it by the deadline below.

This form is for the June 2nd Primary Election.  You must be registered with a party (Democratic or Republican) in order to vote a ballot.  You may only vote for one party's ballot.  If you are currently registered as No Party and you request a ballot for a party, your party affiliation will be changed to that party.  If you wish to change back, you may after June 3rd.  The first day we can accept Absentee Ballot Requests is March 24, 2026. (Any forms recieved or dated earlier than that will be rejected per code.)

Deadline to Return an Absentee Ballot Request Form for the Primary Election:  Monday May 18, 2026 at 5 pm.   Submitting your form earlier is always preferable. Ballots begin mailing on May 13th, 2026.

NOTE: We CANNOT accept requests for a ballot for the November 3rd General Election until August 25th.  Any forms submitted before that are invalid per Iowa Code.
